Moving Home Minnesota Employment Providers

Description: Moving Home Minnesota Employment Providers will be trained to utilize Customized Employment tools and techniques to assist individuals with disabilities acquire individualized competitive employment* positions in community businesses. Selected individuals must successfully complete the 40-hour ACRE Certified Customized Employment Training developed by Griffin-Hammis Associates, LLC. This includes attending all sessions and passing the post-test with a score of 80% or better.

Customized Employment is a flexible process designed to personalize the employment relationship between a job candidate and an employer in a way that meets the needs of both. It is based on an individualized match between the strengths, conditions, and interests of a job candidate and the identified business needs of an employer. Customized Employment utilizes an individualized approach to employment planning and job development — one person at a time – one employer at a time.

Customized Employment consists of 4 phases, each of which is connected to each other. All four phases; Discovering Personal Genius™, Cultivating Employment Opportunities, Engaging Employment Supports, and Ongoing Support and Career Development, must be completed to assure the best employment situation that fits the job seeker best and meets the needs of the business or employer.

Commit to providing Customized Supported Employment Services to MHM participants through the MFP demonstration grant. (Five years beginning January 2013).

  • Attend and successfully complete the 40 hour ACRE Certified Customized Employment Training (attend all sessions and pass post-test)
  • Complete Discovery Staging Records (DSR) and accompanying documentation/forms and submit first three to MNTAT for review during development and upon completion
  • Participate in monthly web or phone conferences with all Employment Providers and MNTAT/GHA consultants
  • Attend 2-day follow up training (dates to be determined)

Agree to the following process conditions:

  • A signed agreement for the individual, (include employer or supervisor area, if applicable) indicating a commitment to do the work of Money Follows the Person, providing employment services and to participate in an advisory role to MHM; also available resources like a car and computer.
  • One person at-a-time
  • Discovery Assessment = 6-8 weeks (30-60 hours)
  • Develop Themes
  • Create Lists of Twenty
  • Conduct Informational Interviews (3-5 per week)
  • Secure wage employment or pursue self-employment (when indicated) for each assigned job seeker
  • Complete worksite analysis and training plan upon hire
  • Report Data/Outcomes – (hours of Discovery Process & Job Placement – how much time face to face, on behalf of the person and administration) also earned income, hours worked per week, longevity – employed vs. individual job, impact on benefits.
